Cover Story: Cement augmentation of cephalomedullary head elements is used to improve fixation in osteoporotic trochanteric fractures, but it does not remove the need for accurate positioning and preserved sliding. This cover highlights a rare failure: a post-union subcapital femoral neck fracture around a cement-augmented helical blade. Despite acceptable initial metrics, sliding remained constrained and progressive varus displacement developed, redirecting load to the subcapital region rather than dissipating it through telescoping. Serial radiographs with complementary CT/MRI suggest that excessive construct stiffness can shift stress after union. We propose a position- and sliding-based guide to flag early warning signs—pain, progressive varus, and lack of sliding progression—and to prompt consideration of conversion to arthroplasty.
