Cover Story: Carotid endarterectomy is a common procedure performed to reduce stroke risk in patients with carotid artery disease. However, a higher common carotid artery bifurcation height can increase operative difficulty and the risk of cranial nerve injury, particularly to the hypoglossal nerve. Current methods for classifying bifurcation height often rely on deep anatomic landmarks and advanced imaging, limiting their clinical utility. In this study, we examined the relationship between the height of the common carotid artery bifurcation and easily palpable anterior neck landmarks. We demonstrate a strong association between bifurcation height and proximity to key surgical structures, providing a practical framework for the identification of higher-risk anatomy. When combined with ultrasounds, these findings may improve preoperative assessments prior to carotid endarterectomy. View this paper
